Registration for the 2025/26 sports season is OPEN!
Click Here to Register on FinalForms! You only need to register ONE time for the whole year! Just make sure to select every sport you are interested in playing.

ELIGIBILITY
In order to be eligible for Athletics, you must complete the following 3 items:
- REGISTER for your sport using Final Forms.
- Parents AND Students must sign forms in Final Forms registration
- *Please use student’s username@mead354.org email address when registering
- NOTE: if students login through a browser, they will authenticate through Microsoft (formerly HelloID) which prompts them to use their username@msd354.org and the same password.
- Have a CURRENT PHYSICAL uploaded to Final Forms or on file with the Athletic Office - sports physicals are good for two years.
- PAY YOUR FEES: HMS students-athletes are required to pay a one-time $35 Participation Fee and $15 ASB Fee
- Fees can be PAID ONLINE HERE or you can pay at our Business Office.
